Whats wrong with my hippo?

My Tang has been battling ich for a while now. White spots seem to come and go. He's eats and is active. I've talked about him in my previous thread.

So here is some pics of this new thing that has come up lately and hasn't fought it off as well.

It looks like black cysts.

What is wrong with him and how do I fix it?

I took a video, hopefully it give you a better idea. Do you still think it's black ich? The area that concerns me is the big white spot in the middle of his upper black part. It seems to big to be ich. All your input is appreciated!
