That is not doing well at all it is dying this happend to mine in one day the 3 things that were wrong were temp to high, nitrate spike around 40ppm, phosphates to high I can't remember the ppm
Only one polyp survived I fraged it and it's recovering nicely now.

The temp is 80. Nitrites are 0 and nitrates are 0. Ph is 8. Numbers have been steady fir months. What's weird is the green star is fibe

Check phosphate. I had a tank that softies did really well in, but LPS floundered. The only thing off was phosphate.
