Coral help needed!!

Does anyone know what type of coral this is? Also where it should be in my tank etc. I was given it a couple of weeks ago and have tried it both low and high in my tank but it seems to be dying off wherever I put it. My water levels are fine, I have two t5 lights which should be strong enough, or does it not need much light?

I'm a beginner when it comes to coral so any advice would be great.

It is a favites,(pineapple coral) LPS med light flow-typically hard to kill-need more specifics about tank and water parameters
