Chevron Tang. I see them for sale quite often. Pacific Island Aquatics has them right now if you really want to get one. They have a website with all sorts of cool fish.

The juninile coloration is stunning on those fish, but the color changes to a much less impressive brownish black as they age

They are nice looking fish though. Anderson14 has one in the 'Twins'

Definitely one of my favorite tangs, probably tied with the purple tang for its beauty!
