If it is opened up and extracting its polyps it's most likely healthy.

It looks to be branching and has good color. I have one about the size of a soccer ball when fully extended. You can see part of it in my avitar.

T-5's are fine for either. MH's will work too but will need to be acclimated to the stronger light.
